Due at time of each service:
  • All co-payments, deductible payments, and non-insured payment for fees and services must be paid at
    each session.
Please verify your health insurance coverage before scheduling your first visit - ultimately you are
responsible for all fees, insurance companies do not guarantee payments

There is a 25$ cancellation fee for failure to cancel 24 hours in advance by 7 a.
m. the of day your are scheduled. This fee must be paid at or before your next
scheduled appointment.
